We
are looking for a full time Health and Wellbeing Coach to work with the 5
practices across the network (Holsworthy, Hatherleigh, Stratton, Neetside and
Bradworthy).

Main duties of the job
The
role involves coaching and motivating patients in managing their own health and
wellbeing, to live independently and improve their health outcomes. The post
holder will work with individual and/or groups of patients and liaise with
other healthcare providers. The post holder will work for HB&SV PCN as part
of their multidisciplinary healthcare professional teams; support and
supervision will be provided by a GP Supervisor and/or a line manager. This is
a key position within our developing PCN and appropriate training will be
given. Because this role may involve travelling between our sites, a full clean
driving licence is essential.
