Job summary
We are looking for a Senior Clinical Pharmacist to join our multidisciplinary team and play a key role in the safe and effective use of medicines across the practice and wider Primary Care Network.
This is a senior clinical role with responsibility for supporting patients with a wide range of conditions, including those with complex needs and long-term conditions. The post holder will manage their own clinical caseload, undertake structured medication reviews, and support patients to get the best outcomes from their medicines.
The role also involves working closely with GPs, nurses, and the wider team to support safe prescribing, respond to medicines queries, and contribute to the management of high-risk medicines and hospital discharge processes.
As a senior member of the pharmacy team, you will support and guide other pharmacy staff, helping to develop skills, improve systems, and strengthen medicines optimisation across the practice.
You will also play a key role in service development and quality improvement work, helping to review and improve prescribing processes and support better patient care.

You will be supported by the Clinical Lead Pharmacist and GP Partners, with opportunities for ongoing development and progression.
Main duties of the job
The Senior Clinical Pharmacist will play a key role in supporting safe, effective, and evidence-based use of medicines within the practice and wider PCN. They will work directly with patients, managing a varied caseload and providing clinical reviews, advice, and follow-up for patients with acute and long-term conditions.
They will support the safe and effective management of medicines across the practice, including high-risk drugs, repeat prescribing processes, and medicines reconciliation following hospital discharge. A key part of the role will involve responding to medicines queries from patients and staff, helping to resolve issues and improve understanding and adherence.
The post holder will work closely with GPs, nurses, and other members of the multidisciplinary team to support safe prescribing and contribute to the care of patients with complex needs. This will include supporting long-term condition management clinics and contributing to patient care planning.
They will also take an active role in improving prescribing quality, supporting audits, and contributing to service improvement work.
As a senior member of the pharmacy team, they will provide guidance and support to other pharmacy staff and help develop systems and processes that improve patient safety and service efficiency.
They will also contribute to education, training, and the development of pharmacy services across the organisation, working collaboratively with internal and external healthcare partners.
