Main duties of the job
Experience is welcomed, but not essential and salary is negotiable
depending on experience.
The role is for 27 to 37.5 hours per week, andwill involve assessment and management of patients, which will
include a mixture of same day appointments as well as planned appointments and
telephone triage. Our priority is finding the right person as we continue to
develop our services for our patients, and the successful candidate will be
integral to this.

The post-holder will work within the Advanced
Care Practitioner team to deliver excellent primary health care services within
the practice.

The post-holder will work collaboratively with
the multi-disciplinary general practice team to meet the needs of patients,
supporting the delivery of policy and procedures.

The post-holder will build effective working
relationships with other health care professionals.
- Provide
a first point of contact within the practice for patients presenting with
undifferentiated, undiagnosed problems, making use of skills in history
taking, physical examination, problem-solving and clinical
decision-making, to establish a diagnosis and management plan.
