Social Prescriber Link Worker Specialising in Substance Misuse

Job summary

We have an exciting opportunity to join our Social Prescriber link-worker team in Torbay. There are 2 roles available, one working with the drug and alcohol team and one with the probation team. These link worker roles are unique as they will involve empowering individuals with substance misuse to take control of their health and wellbeing. The successful candidates will be helping individuals to strengthen their personal resilience through connecting with community groups and statutory services for practical and emotional support.

This is full time position, 30 -37.5 hours per week over a fixed term 12 month period. Salary is £22,000 - £25,000 per annum, pro rata (depending on experience). About us

You will be based in GP practices in Torbay, working across several different practices, and with the drug and alcohol and probation services, as well as completing home visits as necessary. You will be embedded as a member of the practice teams, receiving full support and training in your role from the existing Social Prescriber link workers, specialist substance misuse services and local training providers. A qualification and or/interest in mental health is desirable.
