Job summary
You will join a group of GP practices in Torbay Compass House Medical Centres, Pembroke Medical Group and Chilcote Surgery. You will be part of an experienced Clinical Pharmacist team, supporting our Practice based pharmacists to ensure the delivery of safe, effective & efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. The role will also include training/development of practice based prescribing co-ordinators and administrators to support improved co-ordination and effective pharmacy administration within general practice. The role is 37.5 hours per week & salary is £22- £30,000 per annum depending upon experience. Main duties of the job

reconciliation in accordance with practice protocols and to update patients
medical records / prescribing accordingly.To support the achievement of

Working with the Clinical
Pharmacist and Prescribing Co-ordinators to contribute to the review and
development of repeat prescribing protocols / SOP's to
improve the efficiency and effectiveness of repeat prescribing.
To assist in the appropriate
monitoring management & audits of prescribing budgets and to liaise with the
medicines management team at the Devon CCG.
To support the Practice Based
Clinical Pharmacist to provide medication reviews, patient information
leaflets, medicine awareness; and to undertake community visits as appropriate.
To respond to medication queries
from patients and staff in a professional manner referring to the appropriate
GP in accordance with practice protocols.
