Exciting opportunity for a Practice Nurse to join our supportive team

We are seeking a motivated and enthusiastic General Practice Nurse to join our friendly and forward-thinking primary care team. This role offers the chance to deliver high-quality, evidence-based care across a wide range of services, including chronic disease management, treatment room duties, health promotion and preventative care. You will work both independently and collaboratively as part of our multidisciplinary team, contributing to excellent outcomes for our diverse patient population.

We welcome applications from nurses with varying levels of general practice experience:

  • Experienced practice nurses who can demonstrate established skills in general practice nursing and chronic disease management.

  • Nurses who are new to general practice or have early experience in long-term condition management, who will be supported with a structured induction, mentorship, and a tailored development plan.

Main duties of the job

  • Provide general treatment room services including wound care, cytology, immunisations, travel health, venepuncture, ECGs and ear care.

  • Run chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension, CHD) in line with local and national guidance.

  • Support delivery of QOF, IIF and enhanced services.

  • Assist with minor surgery and womens health procedures.

  • Promote health education and self-management, offering lifestyle and preventative advice.

  • Actively contribute to infection prevention and control, clinical governance, audit and service development.

  • Work closely with colleagues across the practice and wider community to provide holistic, patient-centred care.

    Pendleside Medical Practice is a friendly, well-established and respected GP Practice based at Clitheroe Health Centre in the centre of Clitheroe. We are a teaching and dispensing practice rated outstanding by CQC at our last inspection and consistently high achievers in QoF.

    We are a progressive GP practice serving a diverse patient population of 11900 patients who live in clitheroe and the surrounding area. We strive to provide high-quality, compassionate care while fostering a supportive and collaborative working environment for our staff. We are part of Ribblesdale Primary Care Network and work closely with local community and secondary care services.     Job responsibilities

    Key Responsibilities Clinical Care
    • Provide treatment room services including: cytology, wound and ulcer care (including Doppler), suture removal, ECGs, venepuncture, ear care, immunisations (adult and childhood) and travel health.

    • Run chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension, CHD) in line with current guidance.

    • Support new patient health checks, urinalysis and preventative health screening.

    • Assist GPs with minor surgery and womens health procedures (e.g. coil fittings).

    • Act as a chaperone when required.

    • Request, process and act upon pathology tests as appropriate.

    Professional Standards & Governance
    • Work within the NMC Code of Conduct and practice policies.

    • Maintain accurate and contemporaneous patient records.

    • Contribute to QOF, IIF and enhanced service delivery.

    • Ensure safe storage, use and disposal of vaccines, medicines and clinical equipment.

    • Take a lead role in infection prevention and control, maintaining safe clinical environments.

    • Participate in clinical audit, significant event reviews and service development.

    Health Promotion & Patient Education
    • Provide health education and lifestyle advice, including smoking cessation, weight management and contraception.

    • Support patients to self-manage long-term conditions.

    • Order and maintain health promotion resources for patient use.

    Team Contribution
    • Work collaboratively with colleagues across the practice and wider PCN.

    • Support and mentor healthcare assistants and junior staff where appropriate.

    • Attend practice meetings, participate in appraisal and contribute to training programmes.

    • Relevant post-registration experience in primary care or community nursing.
    • Competence in cytology, immunisations and chronic disease management.

    • Additional training or experience in diabetes, CHD or Asthma/COPD.
    • Experience of audit and quality improvement.
    • Mentorship/teaching qualification.
