Job summary
We are
looking to appoint a caring and committed, highly motivated, experienced Chronic Disease Nurse to
join our established team of 2 treatment room nurses, 1 chronic disease nurse, Nurse Associate and HCA . This vacancy is for approximately 30 hours per week, to be discussed at interview.
Main duties of the job
The
candidate must be experienced with respiratory knowledge and management of patients
with asthma and COPD. ARTP certificate of competence desirable but further training would be available.
In addition we are looking for experience in Diabetes management including initiation of GLP1 and Insulin.
A qualification as a nurse prescriber would be an advantage.
Competent
to maintain responsibility for patients with long-term conditions delivering evidence-based
practice in conjunction with QOF targets. Self-directed and ability to work
autonomously whilst liaising with other members of the practice and
multi-disciplinary team.
About us
We
are a friendly practice serving a population of approximately 12,500 in a
semi-rural area. Our main site is a
large modern purpose-built practice which is also shared with physiotherapy,
chiropody, community services and other multi-disciplinary services. We also have
a small second site in Meriden which is 3 miles away
