Advanced Nurse Practitioner / Paramedic Practitioner Health Inclusion Project Clinical Lead Brunel Healthcare Network PCN

Are you an experienced ANP or Paramedic Practitioner with a passion for health equity and inclusion? Do you have a strong clinical background in primary care/prehospital settings, particularly with complex and marginalised populations?

We are looking for a dynamic and compassionate clinical leader to drive forward an innovative Health Inclusion Project at the heart of Portsmouth City. This is a unique opportunity to shape and expand care delivery for the city's most vulnerable populations, including those experiencing homelessness and the travelling community.

Main duties of the job

As our Health Inclusion Project Lead, you will play a pivotal role within the Integrated Neighbourhood Team, strategically and clinically leading a dedicated service aimed at improving access to high-quality, tailored care for people who often fall through the gaps.

You will:

  • Lead and develop a small, passionate team providing outreach and primary care to marginalised patients.
  • Identify gaps in service provision and work closely with health, social care, and voluntary sector stakeholders to co-design innovative solutions.
  • Advocate for patients and drive equitable access across systems.
  • Provide assurances of outcomes to the PCN
  • Bring clinical expertise and leadership to complex case management in non-traditional care settings.

Lead the planning, deliver and evaluation of the Health Inclusion Project across the PCN and wider community settings.

Provide high-quality, autonomous clinical care including assessment, diagnosis, treatment and prescribing (where qualified).

Engage with patients in various outreach location- such as hostels, shelters, community centres and mobile clinics, to deliver responsive and flexible care.

Work closely with partner organisations including mental health services, housing support, substance misuse services and voluntary sector agencies.

Identify gaps in access or provision and advocate for service improvements and innovation.

Educate and support colleagues on inclusion health principles and trauma-informed approaches.

Monitor clinical outcomes, contribute to service audits, and support quality improvement initiatives.

Promote a culture of compassion, collaboration and equity throughout the PCN

Qualifications

Essential

  • NMC or HCPC Registered.
  • Advanced Clinical qualification at Master's level (or working towards).
  • Independent Prescriber (V300 or equivalent) or working towards.

Experience

Essential

  • Proven experience delivering care autonomously in primary care, urgent care or community settings.
  • Strong commitment to tackling health inequalities and supporting underserved populations.
  • Excellent communication, collaboration and leadership skills.

Desirable

  • Experience of working with marginalised or socially excluded populations such as homeless individuals or the travelling community.
