Job summary
Our Primary Care Network (Brunel PCN) are excited to offer a unique opportunity for an experienced Advanced Nurse Practitioner (ANP) or Paramedic Practitioner (PP) to lead and drive forward our innovative Health Inclusion Project; delivering equitable and accessible healthcare to some of our most vulnerable community members - including those experiencing homelessness and individuals from the travelling community.
This is a pivotal leadership role alongside our proactive Integrated Neighbourhood Team (INT), working across the Primary Care Network (PCN) and in partnership with a wide range of community services. You'll be instrumental in shaping inclusive, person-centred care and addressing health inequalities within our neighbourhoods.
Main duties of the job
Lead the planning, delivery and evaluation of the Health Inclusion Project across the PCN and wider community settings, providing high-quality autonomous clinical care including assessment, diagnosis, treatment and prescribing (where qualified).
Full UK Driving licence and access to use of own vehicle is essential for this role.

- Significant post-registration experience in a senior clinical role.
- Experience working in primary care, community, or outreach settings.
- Proven ability to work autonomously in clinical assessment, diagnosis and treatment.
- Experience of working with marginalised populations (e.g. homelessness, travelling communities, asylum seekers).
- Experience of supervising or mentoring clinical staff or students.
- Experience of leading or contributing to service development or innovation.
Knowledge and Understanding
Essential
- Advanced understanding of physical and mental health conditions and complex care needs.
- Strong knowledge of health inequalities and inclusion health principles.
- Sound understanding of safeguarding procedures and trauma-informed care.
- Familiarity with multidisciplinary and multi-agency working.
