Job summary
Our Primary Care Network (Brunel PCN) are offering an exciting opportunity for a dedicated Mental Health or Dual Trained Nurse to join our innovative Health Inclusion Team;who provide vital care and support to individuals who are often marginalised within the healthcare systems - including people experiencing homelessness, members of the travelling community and others facing significant health inequalities.
This Band 6 role is initially offered as a 1 year fixed-term contract, with the possibility of moving to a permanent contract. Hours per week are 30 to 37.5 to be negotiated.
You will work across the Primary Care Network (PCN) and within the community, delivering holistic, trauma-informed care in flexible, non-traditional settings alongside a skilled, compassionate multidisciplinary team.
Full UK Driving licence with access to own vehicle for work purpose is essential for this role.

Main duties of the job
Provide outreach care in a range of community settings such as hostels, outreach hubs and temporary accommodation units, supporting individuals to access healthcare and community services, advocating on their behalf where appropriate.
