We are seeking a Salaried GP with the option to become a Partner, for the right candidate, for 4 to 8 sessions per week.
East Shore Partnership is an inner-city practice
based in Portsmouth, Hampshire. It is within easy access to road/rail links and
good local schools and colleges. We are a friendly and forward-thinking
practice which serves over 13,500 patients within the local area.
We have a good CQC rating and excellent QOF achievement and are a member of Brunel Primary Care Network.
We are an established GP training practice for vocational training and offer training support for nurses and pharmacists too. We are committed to the professional development of our staff at East Shore Partnership.
Main duties of the job
The successful candidate will have:
Knowledge and understanding of services relevant to primary care.
Knowledge of current and forthcoming national policy and its effects on clinical practice.
Knowledge of GP contracts, enhanced services and QOF

Our clinical team comprises of 2 GP Partners, 2 Salaried GPS, 2 Advanced Nurse Practitioners, Pharmacist, Pharmacy Technician team, 4 Nurses, 2 HCA, 2 Physician Associates and access to a Diabetes and Respiratory Nurse Specialist; and very supportive and skilled administrative staff who assist in maintaining excellent patient care.

Key Responsibilities and Duties:
Surgery Consultations, triaging
calls as appropriate, occasional home visiting and administration associated
with patient care.
Work closely with
Advanced Nurse Practitioner to run duty sessions, providing advice and support.
Work as a team member
with all relevant providers of primary and community care services within the
practice.
Ensure appropriate
records are maintained at all times regarding patient care and that relevant
team members are kept fully informed regarding patients seen on behalf of the
practice.
Ensure that the practice
achieves the highest possible QOF outcomes.
Developing care plans
for health in consultation with patients and in line with current practice
disease management protocols.
Support the proactive
care agenda by participating in care planning and multi-disciplinary meetings.
Attend team and healthcare
meetings when required and represent East Shore Partnership at external
meetings.
Other responsibilities within the Practice:
- Maintain confidentiality of the practice
at all times.
- Work in accordance with and comply with
all practice policies and procedures/guidelines. E.g. health and safety,
prescribing, GDPR, and assist in their development.
- Fulfil any monitoring requirements
attached to the post as required.
- Actively participate in clinical
governance within the practice as required by the Performance and Practice
Manager.
- Contribute to the development of
computer-based patient records and summarising of patient records and
read-coding patient data.
- Maintain and develop individual skills
and knowledge, ensuring statutory, mandatory and Trust required training
updates are completed in a timely manner. Committed to lifelong learning.
- Attend training and events organised by
the practice or external agencies where appropriate.
