We have an exciting opportunity for a Practice Nurse to join our respected GP practice in St Helens for up to 21 hours per week.
We are a training practice so training and education is at the heart of what we do. We are looking for a nursing colleague who shares our passion for primary care, for education and who cares about the culture of our workplace and whose values are aligned to ours. You will be able, with the right support and training, provide primary care services to our population of 7100 patients, offer direct supervision to our Nursing Associate and help contribute to our educational portfolio by supporting student nurses.

Salary scale: £18.10 - £20.86
We welcome applications from nurses from other sectors who have some generalist experience. Full training in General Practice nursing can be given as well as access to a GPN Fellowship.
Main duties of the job
Provide general
nursing duties for the practice:
Autonomously diagnose, manage and monitor patients
within specified chronic disease areas for example diabetes, hypertension,
COPD, asthma, IHD, CVA and CKD.
Initiate and interpret investigations as required.
Initiating/adjusting treatment as appropriate,
using national and local guidelines/protocols within our area of expertise,
liaising with GPs as required.
Refer as required to district nursing, podiatry
dietetics, primary, secondary community care and the voluntary sector.
Advise and recommend complex, specialised treatment
plans to patients and carers for the management of chronic diseases, ensuring
informed consent with patient/carer.
Initiate and implement national screening and
immunisation programmes appropriate to general practice.
Undertake medication reviews in the chronic disease
categories within set guidelines
Take lead responsibility for achieving maximum
targets for the GMS and Quality contracts as determined, nationally and locally
enhanced services within the individuals specialised area and supporting other
clinical teams as appropriate
Conform to health and safety and control of
infection regulations
Act in accordance with Nursing and Midwifery
Council Code of Professional Conduct
Maintain legible timely and concise documentation
and input clinical information to the practice computer database.
Undertake home visits for flu and pneumo
vaccinations and appropriate health checks

Identify, manage and support patients as risk of
developing long-term conditions, preventing adverse effects on the patients
health
As required, provide routine nursing care to patients in
accordance with clinical based evidence, NICE and the NSF
Review patient medications to enhance compliance
Coordinate the provision of travel medicine services
Request pathology services and process pathology results
as required
Provision of chronic disease clinics,
delivering patient care as necessary, referring patients to secondary /
specialist care as required
Maintain accurate clinical records in conjunction with
extant legislation
Ensure read codes are used effectively
Ensure the appropriate maintenance and use of chronic
disease registers
Ensure compliance with the practice chaperone policy
Prioritise health issues and intervene appropriately
Support the team in dealing with clinical emergencies
Vaccination programmes for adults and children
Support patients in the use of their prescribed medicines
or over the counter medicines (within own scope of practice)
Liaise with external services / agencies to ensure the
patient is supported appropriately (vulnerable patients etc.)
Support the clinical team with all safeguarding matters,
in accordance with local and national policies

Support and participate in shared learning within the
practice in order to improve patient care
