Main duties of the job

As a GP with Deal and Sandwich Primary Care Network, you will play a vital role in delivering patient care, following the GMS contract and Enhanced Services objectives. Key responsibilities include:

Conducting routine and urgent appointments, both face-to-face and via telephone, ensuring high-quality care.

Prescribing according to local and national guidelines.

Handling home visits for routine and emergency cases.

Managing administrative tasks, such as letters, results, referrals, and emails, in a timely manner.

Making appropriate referrals and managing related admin.

Checking and signing repeat prescriptions to maintain care quality.

Providing cover for colleagues in emergencies, considering individual commitments.

Participating in on-call sessions as per pro-rata hours.

Supporting Extended Hours services, enhancing patient access.

Collaborating with other health and social care providers to deliver integrated care.

Maintaining accurate consultation records using the EMIS clinical system.

Offering clinical support to colleagues, helping the team meet high standards in patient care and QOF/Enhanced Services targets.

This is a dynamic role that offers both patient care and team collaboration in a supportive environment.

Deal & Sandwich Primary Care Network (PCN) comprises 5 GP practices: Balmoral Surgery, Cedars Surgery, Manor Road Surgery, Sandwich Medical Practice and St Richards Road Surgery, with a combined list size of around 48,000 patients. The practices work together to deliver care and support through new and innovative ways which enables our patients to live healthy and fulfilling live.

Clinical responsibilities:

In accordance with the practice rota, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.

Providing counselling and health education.

Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreed standards.

Collecting data for audit purposes.

Expectation to complete a mandatory induction.

Compiling and issuing computer-generated acute and repeat prescriptions.

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.

The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated.

The post-holder will undertake a fair percentage of enhanced access sessions as allocated on the network rota.

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including participating in shared learning across the practice, PCN and wider organisation. Work as part of the team to seek feedback, continually improve the service and contribute to business planning.

Use technology, read codes and appropriate software as an aid to planning, implementing and monitoring care, communicating information and reviewing and processing data for audit purposes.
