Job summary
Monkfield Medical Practice is seeking an experienced and
motivated Diabetes Nurse to lead and deliver high quality diabetes care for
patients with Type 1, Type 2 and prediabetes. The post holder will work
autonomously within primary care, providing assessment, diagnosis, treatment
planning, medication titration, and ongoing review.
A part of this role will also be to educate and upskill the
wider clinical team, ensuring all staff maintain strong diabetes knowledge and
competencies. The post holder will also contribute to maintaining high QOF
performance, particularly in diabetes indicators, and may support additional
cardiovascular QOF areas such as hypertension, CHD and heart failure.
The position holder will promote the health and wellbeing of
people with diabetes enabling them to achieve optimum Glycaemic, Blood
Pressure, Cholesterol and other key parameters. In this they may identify other
conditions or abnormalities and escalate these appropriately internally or by
referral.
The role would be part of the nursing team under the
leadership of our lead nurse, and the clinical partners.
We would expect the successful candidate to integrate and work with the nursing
team and other staff in a flexible and mutually beneficial way.

Clinical Care
Run
autonomous diabetes clinics to assess, diagnose, plan, implement and
evaluate care for patients with Type 1, Type 2 and prediabetes.
Initiate,
titrate and monitor diabetes medications, including oral hypoglycaemics,
GLP1s, SGLT2s. Insulin initiation is not essential but titration is
required.
Provide
structured diabetes education and selfmanagement support
Identify
and manage patients with complex needs, including those with
multimorbidity.
Request
and interpret diagnostic tests and communicate results appropriately.
Recognise
and act on signs of undiagnosed or worsening conditions.
Undertake
home visits where clinically required.
Quality and QOF
Maintain
high QOF performance across diabetes indicators.
Support
QOF delivery in related cardiovascular areas (e.g., hypertension, CHD,
HF).
Ensure
accurate coding, register management and recall processes.
Participate
in audit, service improvement and clinical governance.
