Job summary
Auckland Medical Group are looking to recruit an experienced and motivated Chronic Disease Nurse.
The post holder will provide and maintain a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and developement of individual management plans and agreeing these as appropriate with the patient and other health professionals.
The post is for 24 hours per week.
Main duties of the job
To join our nursing team and be responsible for chronic disease management of our patients.
The successful applicant must have excellent communication skills and flexibility is imperative due to the nature of the role
About us
A busy teaching/dispensing practice
with over 15950 patients based over 3 sites. We have 6 partners, 2 salaried GPS, 4 nurse
practitioners, 9 practice nurses, 2 nursing associates, 3 HCA, 1 phlebotomist, an in house pharmacy team and a good team of admin and
receptionists.

abnormalities using a holistic approach to history taking and observation
and assessment of physical systems
- Diagnosis, monitoring and
development of individual management plans; agreeing these as appropriate
with the patient and other health professionals
- Understand the aims of management
and the pharmacological treatments for chronic diseases
- Initiate, perform and interpret
diagnostic procedures as appropriate for chronic disease management
- Support recall procedures,
appropriate literature and health promotion, and audit

- Diagnose
and manage both acute and chronic conditions, integrating both drug-and
nondrug-based treatment methods into a management plan.
- Prescribe and review medication for
therapeutic effectiveness, appropriate to patient needs and in accordance
with evidence-based practice and national and practice protocols, and
within scope of practice.
- Provide
information and advice on prescribed or over-the-counter medication, medication
regimes, side-effects and interactions
- Work with patients in order to support compliance with, and adherence to,
prescribed treatments
