seen as essential to the quality of services we provide to our patients. Wellspring Medical Practice is a Teaching Practice with a range of clinical professionals.
Clinical Pharmacists in primary care enjoy a varied role as part of a
multi-disciplinary team. Perform structured medication reviews for patients, especially for those with frail and/or with multiple
co-morbidities along with managing medicines, transfer of care and safer
prescribing. The pharmacist will work closely with our doctors within the practice daily. The Clinical Pharmacist will have a strong
clinical interest.
Main duties of the job
The post holder will support Wellspring Medical Practice in the
North West Primary Care Network.
The post holder will act within their professional boundaries, working
within the pharmacy team.
The successful candidate will work as part of the multi-disciplinary
team and will provide a patient-facing role. They will take responsibility for
areas of chronic disease management and undertake structured medication reviews
to proactively manage patients with complex polypharmacy.
Provide expertise in medicines advice and will support general practice
staff with regards to prescription and medication queries.

Wellspring Medical Practice is part of a large successful PCN and
pharmacy team consisting of 9 GP surgeries that are working together with other
health & social care providers and voluntary services with the aim to
transform primary care in a sustainable way that positively impacts on the
health and wellbeing of the local population.
The practice team is becoming increasingly diverse
incorporating a great mix of professionals including Physician Associates and Clinical Pharmacist.

the safety and quality of care for patients and

To contribute to quality improvement and clinical audit, as well as supporting aspects of the Quality and Outcomes Framework,
medicines safety, and antibiotic stewardship.
To contribute towards
practice financial stability
through medicines optimisation and related targets
in QOF.
