Nettleham Medical Practice has an exciting new role for a dedicated and passionate Respiratory Nurse to join its friendly practice team.

Main duties of the job
The post holder will assess, diagnose and review patients who have respiratory conditions providing the appropriate treatment, care planning and self help information as appropriate.
About us
We are a semi-rural practice situated 3 miles north of Lincoln and currently has approximately 12,500 patients. There are two sites, with Nettleham being the main site and a branch surgery at Cherry Willingham.
We have an extensive multi-disciplinary primary care team comprising of GPs, Physician Associate, Nurses, Healthcare Assistants, Phlebotomists, Clinical Pharmacists, First Contact Physiotherapist and Primary Care Chaplains.

Job Summary:
To act as a specialist nurse in the assessment, treatment and review of patients with respiratory disease (COPD and Asthma) providing full holistic care for these patients.
The role would also include being actively involved in research projects in the practice.
Job Responsibilities:
- To assess, plan, implement and evaluate all aspects of patient care including, QoF, referrals to specialist pathways and developing individualised treatment/care plans.
- Conduct Annual Long Term Condition (Respiratory) reviews.
- Offer health promotion advice and support to patients with respiratory disease including smoking cessation where appropriate.
- Promote and encourage self-management for patients living with respiratory disease including provision of personalised self-management plans for patients where appropriate.
- Educate and empower patients and carers to identify early signs of change in their condition and provide them with the necessary knowledge and skills to gain independence and make informed choices to safely manage their condition.
- Administer vaccinations to patients.
- Undertake audits & QUIP programmes for respiratory patients.
- Able to recognize and manage anaphylaxis according to current UK guidelines.
- Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
- Ability to obtain and document informed consent (both verbal or written).
- Able to document Consultations according to NMC guidelines, including use of templates & read codes as per practice policy.
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
- Collaborative working as part of the wider Primary Care Network team which would include extended weekday working and weekend working as part of a rota.
- Confidentially of information gained at work must be preserved at all times.
