Job summary

This new role is to help ensure the seven network practices engage with those patients who are not otherwise engaging with primary care, with a particular focus on health inequalities.

Core20PLUS5 is a national NHS England approach to inform action to reduce healthcare inequalities. This strategy recognises that 20% of the most deprived population, plus those with additional risk factors, have worse health outcomes in five key clinical areas.

The successful candidate will play a key role in complementing our current care coordinators, nurses, clinical pharmacists and IGPC's multi-disciplinary team. This evolving job role has huge scope for development as we expand our PCN team further over the years.

Main duties of the job

  • You will work across the network improving access for those who are not otherwise engaging with usual invitation with a particular focus on, but not limited to, the five clinical areas detailed in the Core20PLUS5 strategy.
  • Case finding and support to access all screening programmes for those with an additional health inequality risk factor.
  • Case finding and support to access chronic disease monitoring for those with an additional health inequality risk factor.
  • Looking at innovative ideas to improve engagement, eg one-stop shop clinics, satellite community clinics, protected appointment slots.
  • Small case holding for a select number of vulnerable individuals, eg someone referred on a 2ww pathway who might otherwise fail to attend a follow up appointment.
  • Engage with network-wide health inequalities projects.

You will be trained to offer basic care during contacts, eg smoking advice, BP monitoring, depression Safety Plans.

You will need to be flexible in your approach to work and have a passion for patient care.

  • Previous experience in a similar NHS role in Primary or Secondary Care.
  • Experience of planning and organising complex meetings/agendas.
  • Evidence of working with IT systems including Microsoft Office.
  • Evidence of ability to support collation and analysis of data.
  • Excellent verbal and written skills.
  • Ability to provide and receive complex information.
  • Excellent interpersonal skills.
  • Evidence of coordinating teams.
  • Evidence of working under own direction.
  • Knowledge of, or lived or professional experience of, disadvantaged groups or those who are discriminated against.
