Job summary
Kensington Partnership is looking to recruit a Practice Nurse.
The post holder is responsible for the delivery of practice nursing care to its registered patients. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring long-term conditions, and engaging in health prevention and screening activities including vaccinations. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Main duties of the job
To deliver a high level of quality care based on the needs of the patient population, which includes treatment, management plans, screening, health promotion and patient education.
Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care, including taking opportunities to promote health and wellbeing wherever they arise
Support and advise women requesting information relating to family planning needs and those presenting for cervical cytology consultations
Implement and participate in vaccination and immunisation programmes for both adults and children- including vaccinations for those travelling abroad

Clinical practice
- To assess the needs of patients ensuring the provision of care is appropriate and incorporates evidence-based practice
- To assess, develop and evaluate individual treatment plans for chronic disease patients, promoting health and wellbeing

Delivering a quality service
