We have an exciting opportunity to join our happy team at West4GPs in Chiswick for a part time or full time salaried position.
We are a forward thinking practice and are keen to develop our working pattern as well as currently utilising new technologies such as online consultations and telephone triage.
We maintain a friendly practice site and a good team spirit. We have a long tradition of teaching medical students from Imperial and Kings Universities. We work alongside allied professionals in our site including District nursing teams.
Main duties of the job
You will take on a role with clinical responsibilities including daily clinics, home visits, administration and participating in the on call rota.
We are currently using telephone triage as well as face to face appointments and using new innovations such as Accurx and e-consultations to run our clinics and see this continuing for the foreseeable future.
We seek a generalist with an interest in chronic disease management. We offer coils and implants in the practice, if this is one of your skill sets we would be happy to support and encourage you continuing this.

Job summary:
The post-holder will manage a caseload and deal
with a wide range of health needs in a primary care setting, ensuring the
highest standards of care for all registered and temporary patients.
Clinical responsibilities:
- Surgery consultations, telephone consultations and
queries, visiting patients at home, checking and signing repeat
prescriptions and dealing with queries, paperwork and correspondence.
- Making professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred from other health
care workers within the organisation
- Assessing the health care needs of patients with undifferentiated
and undiagnosed problems
- Screening patients for disease risk factors and early signs of
illness
- Developing care plans in consultation with patients
- Providing counselling and health education
- Appropriately referring patients for ongoing assessment either in an acute setting or outpatients
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat
prescriptions
- Prescribing in accordance with the NW London prescribing formulary
- In general, the post-holder will be expected to undertake all the
normal duties and responsibilities associated with a GP working within
primary care.
