Part time Senior Management Care Coordinator Job Description
Reports to:Senior GP
JOB PURPOSE
The core purpose of this role is to support the practice's commitment to
deliver integrated, patient-centred care, responsive to the needs of our local
population. The post holder will be a key member of the senior management team,
working collaboratively with colleagues to provide holistic and coordinated
care to our patient community.
JOB SUMMARY
The post holder will be responsible for the co-ordination and delivery
of patient centred care to ensure the achievement of key health and welfare
objectives the practice.
KEY RESPONSIBILITIES:
- Serve as
the contact point, advocate and informational resource for patients, care
teams, family /caregivers and community resources where indicated,
responding with empathy and respect.
- Support
the practice to deliver on quality metrics such as QOF, KPIs and locally
commissioned enhanced services.
- Contribute
to practice capacity, access and improvement projects to meet the primary
care network requirements.
- Support
other management team members, i.e. holiday cover etc.
- Completion
of insurance reports and regular
claims etc.
- Ensure
ARRs claims are submitted in a timely manner and are kept within an annual
budget.
- Maintain
and monitor registers for Vulnerable Adults, Safeguarding Adults and
Children, Veterans, LAC etc .
- Maintain
standards and policies across the practice
in relation to compliance for CQC
- To be
actively aware of reception, administration and secretarial workloads and
cover when required.
- To
organise meetings e.g. clinical/practice , which will include minute
taking and dissemination of minutes
- Ensure
staff trainings are updated and keep robust staff folders
- To comply
with timely parameters in relation to The Medicine Management Team
- To carry
out risk assessments when required
- To
identify and action data quality issues.

The post holder will recognise the valuable contributions of other
colleagues within the organisation and their role to patient care. The ability
to work effectively as a member of a team is essential. The post holder must
also recognise personal limitations and refer to more appropriate colleague(s)
when necessary.
TRAINING AND DEVELOPMENT
- Complete
all mandatory training.
- Actively
participate in annual appraisals.
COMMUNICATION
The post-holder should recognise the importance of effective
communication within the team and will strive to:

effectively with patients and carers.
- Recognise
individuals needs for alternative methods of communication and respond
accordingly.

In the course of seeking treatment, patients entrust us with, or allow
us to gather, sensitive information in relation to their health and other
matters. They do so in confidence and have the right to expect that staff will
respect their privacy and act appropriately. In the performance of the duties
outlined in this job description, the post-holder may have access to
confidential information relating to patients and their carers, practice staff
and other healthcare workers. They may also have access to information relating
to the network or individual practices as business organisations. All such
information from any source is to be regarded as strictly confidential.
Information relating to patients, carers, colleagues, other healthcare workers
or the business of the practice may only be divulged to authorised persons in
accordance with the practice policies and procedures relating to
confidentiality and the protection of personal and sensitive data.

The post-holder must always comply with the Practices Health and Safety

post-holder will comply with the Data Protection Act (1984) and the Access to
Health Records Act (1990).
