Job summary
To work with Primary Care colleagues, patients and carers to navigate the voluntary/
community services environment to address the wider detriments of health and
support social and emotional well-being.
To develop effective relationships with providers of voluntary and
community services and maintain a good knowledge of various programmes offered
locally, aimed at meeting individualised needs.
To maintain regular contact with patients and carers, acting as the
first point of contact if needs change or escalate.
Main duties of the job
To provide personalised, specialist assessment, information,
signposting, support and follow-up for patients to include emotional support.
To
manage and maintain a caseload of patients who have been identified
through proactive case-finding or referred by Practice Staff, ACT, Community
or Social Care professionals, from the network population. Information and
support may be by phone, letter, email, face to face at the patients home
or within their GP surgery.
To
provide information regarding services and benefits available and, where
appropriate, assist patients in accessing them.To be up
to date and familiar with the local or national health, social care and
voluntary sector organisations, as relevant to patients including
maintaining links with local Health and Wellbeing events and linking in
with network forums.
About us
Central Bournemouth Primary Care Network (PCN) comprises four GP Practices, James Fisher Medical Centre, Moordown MC, St Albans MC and the Panton Practice, serving a population of 48,000. The PCN has a well established collaborative approach working with partners in Community, social care, third sector and secondary care.
The successful candidate will join a multi-disciplinary Network team as part of the Self-management staff supported by a Lead Social Prescriber and Health Coach and working alongside Nurses, Pharmacists and Social Care Manager as well as the four Practice teams.
You will be based at the PCN "Hub" at James Fisher MC but work in a variety of Practice and community bases settings and be enabled to work remotely as the needs of the job and requirements for safe working dictate.
