Job summary
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients.
The
post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced
nurse who, acting within their professional boundaries, will provide care for
the presenting patient from initial history taking, clinical assessment,
diagnosis, treatment and evaluation of care.
They will demonstrate safe,
competent clinical decision-making and expert care, including assessment and
diagnostic skills, for patients within the general practice. The post holder
will commit to demonstrating critical thinking in the safe clinical
decision-making process. They will communicate and work collaboratively with
the general practice team to meet the needs of patients, supporting the
delivery of policy and procedures, and providing nurse leadership as required.
They are able to independently
prescribe appropriate medication, evaluate or refer to other specialists if
necessary.
Main duties of the job
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Prescribe and review medication appropriate to patient needs and in accordance with national and practice protocols, and within scope of practice
- Work with patients in order to support compliance to prescribed treatments or over-the-counter medication
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations
- Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation

The following are the core responsibilities of the Advanced Clinical Practitioner (ACP). There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Develop, implement and embed health promotion and wellbeing programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Undertake the collection of pathological specimens
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively
- Maintain chronic disease registers
- Develop, implement and embed well woman clinics
- Develop, implement and embed well man clinics
- Develop, implement and embed an effective call/recall system
- Be responsible for the implementation of an effective immunisation programme
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Implement vaccination programmes for adults and children
- Be an extended and supplementary prescriber, adhering to extant guidance
- Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
- Contribute to practice targets (QOF etc.), complying with local and regional guidance
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual
- Support the clinical team with all safeguarding matters in accordance with local and national policies
- Deliver opportunistic health promotion where appropriate
