Job summary
An exciting
opportunity has arisen for an empowering and action-orientated leader to join
our team. As Clinical Director you will play a vital role in developing the PCN
to meet its key aims and objectives. You will help us to create strategic plans
and improve the quality of services delivered across the PCN by delivering upon
the stated aims of the RPN Steering Group.
Main duties of the job
Work
with practice Clinical Leads to provide clinical leadership for the PCNs
strategic plans, to improve the quality & effectiveness of its delivery of
the PCN DES.
Work
collaboratively & network with other CDs locally &
nationally.
Representing
the PCN at the CD board & ICS meetings.
Being
a voice for the PCN in the wider system.
Participating
in the design of services & system wide plans.
Leading
& supporting quality improvement across member practices (including
professional leadership of QOF quality improvement activity across the PCN).
Strategic
leadership for workforce development through assessment of clinical skill-mix &
development of a PCN workforce strategy.
Supporting
PCN implementation of agreed service changes & pathways.
Working
closely with key stakeholders to support & deliver improvement programmes
aligned to national priorities & the effective management of the PCNs
population health.
To
foster resilience & sustainability of PCN member practices & the health
&f social care economy by promoting out-of-hospital care integration,
efficient ways of working & economies of scale.
Support
work on maximising value (achieving best quality for least cost) in particular
working with the PCN on referral rates, unwarranted clinical variation,
effective prescribing & financial efficiencies.
To
facilitate practices within the PCN to take part in research studies & act
as a link between the network & local Primary Care research networks &
research institutions.
About us
The Rural Practice Network is an established Primary Care Network in South Somerset. We have five member practices - Bruton Surgery, Milborne Port Surgery, Millbrook Surgery, Queen Camel Medical Centre and Wincanton Health Centre. Together we manage the health of a registered population of around 37,000 people. We do this by improving healthcare inequalities, targeting gaps in service provision, encouraging people to pro-actively manage their own health and continuously improving services delivered by GP teams.
We deliver many forward-thinking services in partnership, mostly recently running one of the most successful GP vaccination services in Somerset.
