Job summary

This is an appointment for a Clinical Pharmacist on a fixed term 12 month contract to work within our well established PCN pharmacy team across the Network Area. The role will be based between the neighbourhood's, responding to the needs of the member practices. The post holder will e a pharmacist, who acts within their professional boundaries, providing clinical support within the PCN and working as part of multi-disciplinary teams in general practice. In this role they will receive appropriate clinical supervision. You will ensure that the practices integrate with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare and help manage workload. Manage elements of the quality and outcomes framework and enhanced services such as Quality Improvement. Perform a patient facing role to include undertaking clinical medication reviews to proactively manage patients with complex polypharmacy in a variety of settings including the surgery, patient homes and residential care, and with a view to meeting the demands of new workstreams in primary care. Represent the PCN to strategic partners, as part of a team, on medicines optimisation and quality improvement. Provide primary support to general practice staff with regards to prescription and medication queries, providing expertise in clinical medicines advice. Be supported to develop their role to become a non-medical prescriber if required.

3 Harbours Bosvena is a PCN covering 43,500 patients based in central Cornwall. We have a well established PCN Pharmacy team who cover our 4 practices. We have a diverse population, with pockets of deprivation. We are always working on Health Inequality and Health Management projects and looking at new ways of working to innovate and improve care for our patients.
