Job summary
The Diabetes Specialist Nurse (DSN) will provide care to
individuals with Type1 and Type 2 diabetes requiring specialised diabetes
care.
The DSN will work autonomously in the care and management of
patients with diabetes and in doing so assess, plan, implement, evaluate and
document expert and specialist advice for those patients.
They will work within the Primary Health care Team to extend
current care pathways and protocols and audit the care of patients with
diabetes.
Main duties of the job
The post holder will help coordinate service provision,
providing professional expertise, advice care and support to those needing
treatment for diabetes and their families.
The nurse will work across organisational boundaries and act as a link
by maintaining ongoing contact with patients.
S/he will participate in research and education to advance clinical
excellence in the care of this patient cohort if appropriate.

This partnership consists of 8 doctors responsible
for approximately 17,500 patients in Consett and surrounding areas. The position of Diabetes Specialist Nurse is
challenging and our patient list size generates a heavy and demanding workload.
Applicants should have the ability to work as part
of a team but will also be required to work autonomously.
Applicants should be of smart appearance, with a
pleasant personality. Excellent clinical skills and communication skills are
essential, together with IT experience.
The practice is fully computerised in modern
purpose built accommodation.
