Job summary

The main purpose of the post is the provision of high-quality nursing to the practice population.

Undertake full management of patients including diagnostic procedures, health promotion, collection, and interpretation of results (as appropriate) and therapeutic intervention

Provide general and specific health screening to Practice patients within agreed protocols e.g., cervical cytology, breast awareness, well woman/man checks and new patient registration checks

Advise patients on general health care and minor ailments with referral to GP as and when necessary, either in the surgery or by telephone

Assist in the development of pathways of care in chronic disease management, carry out chronic disease management clinics in accordance with agreed protocols and contribute to the operation of the system for the recall/follow up of patients

Contribute to the development of clinical protocols and templates and undertake audits of chronic disease clinics as and when required

VI. Organise and run nurse led family planning clinics as per protocol, involving other members of the Primary Health Care Team when necessary

Collect and process laboratory specimens as and when necessary

Assist with Minor Surgery as and when necessary

About us

We are a large practice consisting of 12 GP’s supported by an extended team, providing care to approximately 18,000 patients in Consett and surrounding areas. Our premises are a purpose-built Medical Centre.

We are a Training Practice and regularly have a GP Registrars attached through the Northumbria Vocational Training Scheme and Durham and Tees valley GP Training Programme.

Fully computerised – SystmOne, working towards a paper-light system. Computers are used extensively during consultation and as a management tool.

We are a progressive practice, fully staffed, offering a wide range of services, and committed to providing high quality health care.
