Job summary

A new position has arisen within our Data and Care Coordination Team at Pocklington Group Practice.  This is a full time position, 37 hours per week across five days, with a half day during the week.  The hours of work allow for some flexibility with regards to start and finish times.

The role will work closely with our Primary Care Network (PCN) practices (Elvington Medical Practice and MyHealth Group) and the multidisciplinary team in coordinating all key activities including access to services, advice and information and ensuring health and care planning is timely, efficient and patient-centred.

Interviews will take place on Thursday 27 January 2022.

Main duties of the job

Working closely with our Medicines Management Team, this role will act as a point of contact for families, resident and professionals for relevant patients. 

About us

The York East Primary Care Network (PCN) comprises the practices of Pocklington Group Practice, Elvington Medical Practice and MyHealth Group.  Together, the PCN looks after nearly 45,000 patients covering a large, semi-rural area to the East of York.  This role will predominantly be based at Pocklington Group Practice but there may be a requirement to attend other sites from time to time within the PCN.

Job responsibilities

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

· Provide coordination and navigation for people and their carers across health and care services, working closely with Pharmacists, social prescribers, health and wellbeing coaches and other primary care professionals.

· Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care.

· Support the PCN including utilising decision aids and tools where relevant to help primary care professionals to support patients to understand their level of knowledge, skills and confidence (their ‘activation’ level) when engaging with their health and wellbeing.

· Help our patient population to manage their needs through answering queries, making, and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.

· Raise awareness within the PCN of shared decision making and decision support tools, including how to identify patients who may benefit from this.

· As part of the multidisciplinary team, build relationships with staff in GP practices within the PCN, attending relevant meetings, providing information and feedback on care coordination priorities and strategies.

· Liaise directly with care homes and other key providers, and compile and circulate relevant information across stakeholder groups.

· Capture key information to enable comprehensive and accurate records of support, inputting these into clinical systems as required and adhering to data protection legislation

· Liaise with patients for information required to complete their medical notes, which does not require a face to face consultation.

· Run searches to provide quarterly statistics on clinical activity for CCG and Public Health QOF reporting purposes.

· Maintain a diary of required monthly, quarterly, biannual and annual data input to support the practice in ensuring all required financial eligibility is met and ensure it is submitted in a timely manner.

· Support the Operations Manager in the delivery of training to ensure staff can use the EMIS population reporting database to best effect.

· Support the Operations Manager in the delivery of mass annual vaccination clinics including, but not limited to, flu and COVID
