Main duties of the job
Salaried GP role, sessions per week negotiable. Clinical sessions consist of telephone/ video consultations and face to face appointments. Our care navigators are highly skilled to direct patients to the most appropriate service.
About us
Chapelford medical centre is an average sized practice in the Great Sankey area of Warrington. We are a multi-disciplinary clinical and non-clinical team ensuring patients are seen by the most appropriate clinican. Patients can be navigated to multiple roles with the PCN.

- Diagnose and manage both acute and chronic conditions, integrating drug and non-drug based treatment methods into a holistic management plan
- Support patients to adopt health promotion strategies that encourage healthy lifestyles and apply principles of self-care
- Adhere to evidence-based best practice and national guidelines in treating a wide range of clinical conditions
- Maintain clear and contemporaneous consultation notes, to agreed standards, after each patient interaction within the practice clinical system.
- Screen patient for disease risk factors and early signs of disease
- Adopt the highest standards of Information Governance, ensuring that patient data is accessed, shared and stored safely and securely at all times.
- Meet with the practice team regularly to provide clinical advice, embed learning from local/regional meetings and clinical incidents and contribute to the clinical strategy of the practice.
- Provide advice, support and, as required, clinical supervision to other members of the multidisciplinary practice team.
- Attend local/regional clinical meetings on behalf of the practice when required.
- Work with the Practice Manager to manage the priorities of a range of key stakeholders, including the Primary Care Network and the CCG.
- Assist the practice in fulfilling its contractual requirements and achieving key performance metrics, including those specified in QOF, helping to identify opportunities for improving quality of care and the patient experience.
