Job summary

Warrington Innovation Network is a well-established PCN in Warrington, comprising of seven GP practices serving a diverse population of over 50,000 patients. We are a committed and adaptable network who welcome innovation, change and collaboration. This is an exciting opportunity to join our organisation and support the development of healthcare services across a large population.

We are seeking a Care Coordinator with experience in general practice to manage our newly established PCN hub clinical system. You will be required to work across the 7 practice locations around Warrington.

Main duties of the job

The following are the core responsibilities of the PCN Hub Care Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • Maintain and monitor the PCN hub appointment system
  • Process personal, telephone and e-requests for appointments where necessary.
  • Signpost patients to the correct service
  • Initiating contact with and responding to, requests from patients, team members and external agencies
  • Clinically code data on SystmOne.
  • Photocopy documentation as required
  • Data entry of new and temporary registrations and relevant patient information as required
  • Input data into patients healthcare records as necessary
  • Manage all queries from patients and the PCN clinical team as necessary in an efficient manner
  • Carry out system searches as requested
  • Maintain a clean, tidy, effective working area at all times
  • Support all clinical staff with general tasks as requested
  • Maintain PCN clinical rotas along with annual leave

About us

We have a PCN management team which comprised of a Clinical Director, PCN Manager, Practice Manager Lead, Health Inequalities Lead.

We also have a team of Care Coordinators that manage a range of different duties that support our member practices.

Primary key responsibilities

In addition to the primary responsibilities, the PCN Hub Care Coordinator may be requested to:

a. Partake in audit as directed by the audit lead

b. Support administrative staff

c. Action incoming emails and correspondence as necessary

d. Scan patient related documentation and attach scanned documents to patients healthcare records

e. Complete opening and closing procedures in accordance with the duty rota

f. Order and monitor stationery and clinical supplies

g. Produce, maintain and participate in the PCN Hub Care Coordinator rota for lunch and out of hours cover

h. Identify and provide team training where required

i. Support the PCN Administrator in producing and maintaining the duty rota

j. Support the health promotion lead and display promotional material on the allocated noticed boards and in the practice waiting rooms
