Job summary

Responsible for reviewing and consulting with patients remotely; diagnosing and treating patients with existing/suspected cardiovascular risk. These patients will have booked remote appointments at fifteen-minute intervals. There may be a requirement to see extra or emergency patients where necessary. The practitioner will need to prioritise the needs of patients, accordingly, prescribing and advising on medication, making any necessary referrals for investigations in the appropriate manner. There will be a focus on promoting healthy lifestyle and encouraging principles of self-care.

Main duties of the job

Completing clinical reviews via HealthyYou with the support of the clinical record.

Consulting with patients via telephone or video with a view to better management of their hypertension and/or cardiovascular risk.

Completing referrals and signposting patients as appropriate.

Assess therapeutic effectiveness.

Prescribe medication appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.

Work with patients to support compliance with, and adherence to, prescribed treatments.

Recognise and work within own competence and professional code of conduct as regulated by the NMC.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Deliver care according to NSF, NICE guidelines and evidence-based care.

Complete clinical admin applicable to the service.

Review and file investigation results.

Carry out relevant clinical governance activities.

Complete quality audits in line with the audit schedule.

Attend meetings where required, including but not limited to clinical governance meeting and PCN meetings.

Mitigate, investigate and escalate identified risks, incidents and complaints.

Support the wider remote care team including the Operations Manager and Healthcare Administrator where required.

Be a point of clinical escalation for concerns about patient safety.

Assist in onboarding patients to the remote care service.
