Job summary

We are looking to recruit a number of Care Coordinators to work within Queen Square Medical Practice, part of Lancaster Primary Care Network. The roles form an important part of our patient facing service, supporting our Primary Care Clinicians/GPs and acting as an interface with patients, and between them and the PCN, the Integrated Care Community and wider NHS.

The successful candidates will have excellent people and organisational skills and thrive on being part of a team. We hope to recruit individuals with experience of working in the health and care sector or customer service roles; if you have these skills and love working with people, we can give you the training you need. The candidates will play a key role in proactively identifying and working with people, to help them navigate the health system.

Main duties of the job

Care Coordinators bring together all the information about a patients care and support needs and helps to create a personalised service based on what matters to the patient.

They will work closely with all members of the practice team, Primary Care Network Health and Wellbeing Teams, and external organisations to enhance patient care.

To work closely with the patients of the practice and their family/carers to assist with the smooth running of their care and provide continuity of care.

To assist doctors with administrative tasks, working within defined protocols.

Salary: As per contract

Hours: As per contract but ideally Full-time across Mon-Fri

Objectives of the post

  • To perform in an administrative role, to assist the Primary Health Care clinicians, including GPs and Clinical Care Coordinators with the co-ordination of care for patients of the practice.
  • To help provide continuity for patients and a link for patients when Primary Health Care clinicians (especially GPs) are away from the practice
  • To work as part of a team of Patient Care Administrators to support the clinical sub-teams of the Practice.
  • To reduce the administrative workload of clinicians in the practice, working within defined protocols or within clearly defined instructions where protocols do not exist.
  • To assist the practice to perform more effectively.
  • This is a new post within the practice so the job description will be subject to ongoing review until the role is integrated into the practice team.

General Duties:

  • To work closely with all members of the practice team, Primary Care Network, and external organisations to enhance patient care
  • To work closely with the patients of the practice and their family/carers to assist with the smooth running of their care
  • To assist doctors with administrative tasks, working within defined protocols. Duties may include;
    • Review and action normal test results
    • Review and action letters/correspondence that doctors do not need to see
    • Do the administrative aspect of simple referrals on behalf of doctors/nurses
    • Filter requests for information/advice from patients/carers/family/other health care professionals and signpost to them to the most appropriate person to deal with the request
    • This list is not exhaustive and there may be other aspect of administrative work that can safely be dealt with by the care-coordinator.
  • To screen patient information coming into the practice to alert GPs when an action is needed
  • Complete care plan templates on the clinical system
  • To support the needs of patients recently discharged from hospital.
  • To administratively support the management of home visit requests and allocation.
  • To support local patients within care home and nursing homes.
  • Liaise closely with patient and carers on all aspects of care plans
  • Utilise the data and systems available {Urgent Care Dashboard is one example} to alert clinicians when patients may require a care plan review
  • Collect and collate data for audit purposes
  • Attend practice meetings as necessary

Other Duties and Responsibilities:

Awareness of the security of the building and contents, with particular reference to the Health & Safety of self and colleagues, reporting any problems to the nominated representative.

To ensure doors are closed and locked at specific times

To up-date skills as through training/courses recommended by the Practice Manager/Deputy Practice Manager/Team Leader

Dealing with refreshments as and when required

To be available for overtime on a planned and emergency basis

To familiarise yourself with the computer system within the practice to ensure you can carry out your duties to your best ability.

To work effectively and co-operatively within a busy team to ensure the smooth running of the practice.
