We are looking for a Doctor to join our friendly, enthusiastic and innovative 9-partner / 11 doctor practice serving 14,900+ patients. The GPs are supported internally by a great team including;- Nurse Practitioners, Pharmacists/Medicines Management team, Paramedic, Physio, and an extensive Practice Nurse/Health Care Assistant team; and externally by our Integrated Care Community. We pride ourselves on continuity and quality of care, innovation and research, and have strong management and admin support teams.

Our aim is provide high quality care to our patients, whilst maintaining a good work life balance for all of our staff.

We are located in Lancaster situated close to the beautiful countryside of the Lake District, Yorkshire Dales and Morecambe Bay. Lancaster is vibrant university city, and benefits from excellent local state and grammar schools.

Main duties of the job

JOB PURPOSE

To work as a doctor within the Queen Square Medical Practice Primary Health Care Team, comprising of doctors, nurse and pharmacist practitioners, nurses, health care support workers, attached staff (DNs, HVs, CPNs, Counsellors etc.). Holding surgeries and undertaking home visits for patients. Participating in a comprehensive service for the primary care management of acute and chronic disease; and including health promotion, health screening, and illness prevention programmes.

To deliver quality clinical care in accordance with the relevant practice protocols and in line with the GMC Code of Professional Conduct.

DESIRABLE QUALITIES:

FRIENDLY, APPROACHABLE, AND WILLING TO HELP OUT with general jobs through the day (i.e. not just limiting input to surgery and visits.) Examples are undertaking jobs such as answering general queries for staff and patients, taking part in audit, Clinical Governance/education meetings, taking a lead in an area of QOF and/or leading on one or more Enhanced Service where possible.

FLEXIBILITY in working i.e. when given reasonable notice, to be willing to cover occasional extra half or full days. To occasionally cover on-call until 6.30pm. To join in extended hours rota where it falls on your normal working days as Extended hours at present are early morning Monday & Tuesday 7.30-8.00am, Wed evenings 6.30-8.00pm. Also to work occasional Saturday AM (8.00-12.00pm), we currently open every 2nd Saturday of the month and rota is approx 1 every 3mths. NB. These hours are indicative and may change.

COMMITMENT Medium term commitment to Queen Square Medical Practice, ideally 2-3 years minimum, with a general willingness to take part, sharing our ethos and keen to both join in and have a laugh.

To promote good communications within the multi-disciplinary team.

To maintain and update professional knowledge in line with the Clinical Governance Plan, the Practice Development Plan and your personal development plan.

To participate in educational programmes within the Practice.

To maintain standards of conduct and dress to sustain public confidence.

To maintain confidentiality at all times.

CLINICAL RESPONSIBILITIES

To be responsible for assessing, diagnosing, treating and/or referring patients. To opportunistically complete the necessary checks and record appropriate data to enable the practice to meet its targets for such areas as its contract obligations, the Quality and Outcomes Framework (QOF), and other quality indicators from Clinical Commissioning Group (CCG) and Prescribing. Examples include recording patients family history, lifestyle and habits, health status including weight, height and blood pressure, urinalysis and to check tetanus and cervical smear status etc.

To identify, assess and discuss patients problems or needs with the patients, relatives/carers and other Primary Health Care Team members and to devise a management plan and monitor the outcome.

ADMINISTRATION AND MISCELLANEOUS

To complete all statutory and other paperwork/data entry both for medico-legal purposes and to facilitate claims under General Medical Services.

To record all clinical activity and health promotion data in the practice computer system.

To complete Personal Medical Attendant (PMA) reports, for Insurance and legal purposes for patients on your caseload.

All fees earned as a direct consequence of your work for the practice, whether paid under nGMS/PMS or privately, should be paid directly to the practice and do not form part of your remuneration.

To maintain good lines of communication with the partners and the practice management.

To promptly notify the Practice Director or his deputy of any absence.

To book annual leave via the Practice Manager and provide where reasonable to help provide cover for colleagues absence or leave.

To observe health and safety law, and rules as laid down by the practice.
