Job summary

Schoolacre Surgery are looking for an Exeprienced Practice Nurse to join our friendly team

To provide clinical nursing service and support to other professionals and to patients of the practice including treatment, screening, chronic disease management and advice within the professional competence of the post holder. Support and work effectively within the clinical team.

All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regard to the various practice protocols. The nurse is part of a nursing team responsible for ensuring that the practice provides an optimum service of nursing care relevant to the practice population.

Main duties of the job

Main Responsibilities of the Post

Meeting patients nursing care needs identified or work care has been transferred to her by a GP. This will usually be in surgery but occasionally home visits will also be required.

Initiating a mountain health screening a promotional promotion clinic for identified areas of the practice population recognising where modification of systems are referral is necessary

Managing the nursing care of patients with chronic disease employing personal recall system suitable to the practice knees this includes usually practice computer systems for input of information for recall and screening purposes.

Job description

Job responsibilities

Maintaining efficient liaison with all areas of primary care team and other agencies to assure continuity of care when necessary.

Routine treatment in general clinics, including:

Wound management

Blood tests

ECGs

and other general treatment room skills including:

clinical nursing practice

wound management:

Undertake dressing

Suture and clip removal

Screening:

undertake screening including BP checks high and white BMI analysis smoking status

undertake venepuncture according to practice protocol

Health promotion:

Make basic assessments of needs of patients

Provide nutritional advice on:

Weight

CHD

Diabetes

BMI

Infection control

Support patient of colleagues adapting sand infection control measures

Apply infection control measures within the practice according to local national guidelines

This includes:

universal hygiene precautions

the collection handling of laboratory specimens,

segregation disposal of waste materials,

Decontamination of instruments and including clinical equipment

reporting treatment of sharp injuries

dealing with blood and blood fluid apologies

Emergency Management

to be competent in anaphylaxis and resuscitation techniques.

Attend updates when required

Womens Health:

Have a working knowledge of the national and local cervical

Cytology and mammography screen programs

Can discuss self awareness and provide supporting literature to patients

General Duties:

To maintain accurate patient records on the computer

To collect data and participate in regular practice audit and research projects when required

To login dispatch investments to the hospital

To assist the practice nurse in the provision of health promotion and chronic disease management
