General Practice Alliance (GPA) are looking for a Health & Wellbeing Coach to work full time within Grand Union PCN.

This is an exciting opportunity to be part of a role that makes a real difference to people's lives, promoting healthier, more balanced lifestyles.

Main duties of the job

As our Health & Wellbeing Coach, you'll work with people to inspire and support positive lifestyle changes, helping them take control of their health and wellbeing in a way that suits them. You'll bring your experience, understanding, and coaching skills to motivate and guide individuals and groups, adapting your approach to different needs and situations.

Job description

Job responsibilities

Work with referral partners to maximise appropriate referrals and promote the health and wellbeing service.

Deliver personalised health coaching to individuals, focusing on what matters to them and supporting proactive steps toward improving their health and wellbeing.

Use motivational interviewing as a core technique to help individuals make positive lifestyle changes, using open ended questions to explore their values, goals and concerns.

Support habit formation and realistic goal setting, enabling patients to build sustainable routines that promote lifestyle change and long-term self-management.

Facilitate group sessions and workshops that encourage peer support and share learning around the four pillars of wellbeing: sleep hygiene, eating habits, exercise, and wellbeing strategies.

Accurately record and report outcomes using appropriate systems, contributing to service evaluation and continuous improvement.

Manage a caseload of patients, maintaining regular contact and providing ongoing support to track progress, adjust plans, and celebrate successes.

Collaborate with multidisciplinary teams, including GPs, social prescribers, and mental health professionals, to ensure a joined-up approach to patient care.

Promote health literacy and self-care, helping individuals understand their conditions and make informed decisions about their health.

  • An understanding of the different factors that influence our health and wellbeing.
  • Ability to remain resilient under pressure and adapt to setbacks.
  • Able to use a range of tools and techniques to enable and support people, including agenda setting, goal setting, and problem solving.
  • Ability to apply health coaching in both individual and group sessions, using active and reflective listening to build trust and rapport.

Experience

Essential

  • Experience of using coaching approaches, frameworks, models or other helping strategies (e.g. Motivational Interviewing)
  • Experience of supporting behaviour change in individuals or groups.

Desirable

  • Experience of working in health and social care, community development, or a similar setting.

Qualifications

  • NVQ Level 3, Advanced level or equivalent qualifications or working towards.
  • Coaching/counselling qualification or experience or other relevant qualification or experience involving reflective listening skills.

  • Reliable transportation to facilitate a hybrid work arrangement and occasional travel for mandatory meetings as required by the position.
  • Possess the legal right to work in the UK and ability to provide appropriate documentation for verification.
