Job summary

General Practice Alliance (GPA) are the only primary care provider that connects GPs and patients in the Northampton area in a time when medical transformation is inevitable and demands fresh thinking.

GPA are excited to be recruiting a full time Social Prescribing Link Worker to work within our SPRING team. Part time hours will be considered.

Main duties of the job

As a SPRING Social Prescribing Link Worker, your responsibilities are diverse and rewarding, where you will:

  • Initiate client contact and conduct goal-setting sessions to understand and support their needs
  • Create and facilitate wellbeing groups, fostering a sense of community and connection
  • Promote community engagement, empowering individuals to take control of their health journey.
