Job summary

GPA are the only primary care provider that connects GPs and patients in the Northampton area in a time when medical transformation is inevitable and demands fresh thinking.

GPA offers the Collaborative Care Team (CCT) service, designed to assist practices in handling the management of both complex and vulnerable patients, as well as those in need of support during challenging periods.

We are seeking a dedicated Occupational Therapist to join our dynamic team full time on a 12 month FTC!

Main duties of the job

The Collaborative Care Team prioritises out-of-hospital care, aiming to provide support at the patient's place of residence to promote and sustain independent living. The primary goal of CCT is to prevent hospital admissions and facilitate healthy living at home by assessing individual needs and making appropriate service referrals.

As a CCT Occupational Therapist, you will build connections among service users, primary care, and support organisations, and addresshealth inequalities, especially those associated with long-term conditions. The role also involves nurturing independence, facilitating reablement, and promoting effective partnership-working.

  • Demonstrable commitment to professional and personal development

Experience

  • Proficient in completing holistic home assessments and care planning
  • Experience of low-level aids, adaptations and equipment assessing and ordering

Desirable

  • Previous experience using clinical systems such as SystmOne/ Emis Web
  • Experience within General Practice/ Primary Care

Skills and Knowledge

  • Highly effective professional reasoning and communication skills.
  • Excellent organisational skills with attention to detail.
  • Ability to multi-task and remain resilient under pressure.
