a time when medical transformation is inevitable and demands fresh thinking.
We are excited to be recruiting for Health and Wellbeing Coaches to
join our friendly team. This is a 12 month FTC and both full and part time hours are available.
We are looking for individuals who are driven and passionate about making a positive impact on peoples' lives.
Main duties of the job
GPA are working
on two exciting new projects designed to proactively outreach to people in
Northampton with specific long-term conditions, particularly in local areas of
high deprivation.
Our health
coaches will be supporting people to increase their ability to self-manage
their long-term conditions by helping them to build their skills, knowledge,
confidence, and motivation to make healthy lifestyle choices and improve health
outcomes, by working with people to set personal goals and change their
behaviours.
They will be
working with people who have Chronic Obstructive Pulmonary Disease on a
one-to-one basis and will link them into a comprehensive pathway of support
with the aim of reducing exacerbations and hospital readmissions.
Our health
coaches will also be working with people who have indicators, or who have been
diagnosed with coronary heart disease, in group settings, delivering local
Healthy Hearts Community Hubs, to support healthy lifestyle change and
self-management alongside a group of peers.

Health and Wellbeing Coaches manage and prioritise a caseload, according
to the needs, priorities and support required.
They identify when a persons needs are beyond the scope of the health
and wellbeing coach role and refer them back to other health professionals or
organisations.
Health and Wellbeing Coaches may work with people by phone, by video
conference, or face-to-face.
The successful candidate will be kind, reflective and self-aware and
will enjoy working with a wide range of people.
They will have good communication and negotiation skills and a firm
belief that people have untapped resources within them, that can be unleashed
by providing high quality, non-judgemental support.
They will be proactive with a flexible attitude, keen to work as part of
a team and committed to developing themselves in this skilled role and
supporting colleagues to do the same.
Working with referral partners to maximise appropriate referrals and
promoting the health and wellbeing service.
In collaboration with your patients, create personalised plans, measure
outcomes and manage patient information in line with data protection policies.

- Experience of using coaching approaches/frameworks and models or other helping strategies e.g. Motivational Interviewing

- Experience of successful partnership across a variety of organisations.

- NVQ Level 3, Advanced level or equivalent qualifications or working towards.
- Be willing to attend a 4 day training course held by an accredited Personalised Care Institute training company prior to taking referrals
