SPRING is a partnership of General Practice Alliance and
other health and social sector organisations, commissioned to improve the
wellbeing of people in Northamptonshire through Social Prescribing.
We are excited to be recruiting for a full time Social Prescribing Link Worker to work within our SPRING team.
Main duties of the job
As a SPRING Social Prescribing Link Worker (SPLW), you will work as a key part of the SPRING team.
Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical link workers, who support them to identify 'what matters to me'. They take a holistic approach to support and empower people to connect with community groups and statutory services for practical and emotional support.
SPLWs support existing groups to be accessible and sustainable and work collaboratively with all local partners.
We are looking for
individuals who are enthusiastic, self-motivated, empathetic, and strong
communicators who are good at problem solving to join our team. You will be part of a growing team, with
supportive and friendly social prescribers and practice teams.

Primary Duties and Areas of Responsibility
Be proactive, forging relationships and encouraging
referrals from various partnerships in the local community, Community Groups, Voluntary
Sector Services, Primary and Secondary Health and Social Care.
Work in partnership, developing and maintaining relationships
with local voluntary organisations, community, and neighbourhoods to raise
awareness of social prescribing and how partnership working can reduce pressure
on statutory services, improve health outcomes and enable a holistic approach
to care.
Manage a targeted Caseload of people of various
complexity who have Long Term Health conditions.
Giving people time to talk and focus on "What matters
to me". Taking a holistic approach and helping to identify wider issues that
impact on their health & wellbeing.
Creating Personalised Action Plans for individuals using
Wellbeing Stars to support a holistic approach and measure progress on the
journey of change.
Empowering people to take control of their own health and wellbeing
and regain their independence based on their priorities, interests, values
& motivations, using a strength and assets-based approach.
Enable people to achieve their goals by supporting them
to develop and work towards Specific, Measurable, Achievable, Realistic and
time appropriate steps and recognise their achievements.
Working with people on either a 1:1 or group basis,
when required
Where appropriate, physically introduce people to community groups, activities,
and statutory services, ensuring they are comfortable. Follow up to ensure they
are happy, able to engage, included and receiving good support.
Work with commissioners and local partners to identify unmet needs
within the community and gaps in community provision and support development of
new groups and services where needed, through small grants for community
groups, micro-commissioning, and development support.
Understanding, managing & maintaining monthly targets in line with
contractual agreements.
Manage own Data capture and updating of relevant systems in a timely
manner in line with GDPR regulations.
