Job summary

Our Clinical Pharmacists and Pharmacy Technicians are highly valued and well-integrated members of the practice teams, as well as being supported by the wider GPA team. It is due to the success of our existing team that our practices are keen for us to expand. This advert is for full or part time PCN Clinical Pharmacists within the MMWF and Arc PCNs. Please make a note on your application indicating if you would like full or part time hours.

Main duties of the job

The role of the clinical pharmacist in primary care is hugely varied and challenging. We meet regularly with our practices to ensure the pharmacists have clear workplans and objectives. This has been hugely successful in ensuring we are using individual’s skills, delivering on the GP contracts, and supporting the practices as needed.

The successful candidate will be integral part of the practice team, answering medicine queries from patients and colleagues, delivering structured medication reviews, supporting our care home patients with their medicines, working closely with the pharmacy technicians in our team.

We have great links with our local prescribing team, our community pharmacies, the local DGHs, the Primary Care Training Hub and the new ICS. These links mean we can support and work with our colleagues from all sectors to give our patients high quality medicine support.  We have worked with local community pharmacies to introduce CPCS to our local practices and support the use of the New Medicine Scheme referrals. We are passionate about developing pharmacy professionals of the future and currently share both trainee pharmacist and apprentice posts with NGH. We have plans to host undergraduate pharmacy students from the next academic year. All our team support in training new staff, other healthcare professionals and students.

Job Summary

The launch of the new Five-Year GP contract by NHS England emphasises the importance of Clinical Pharmacists in Primary Care. New posts have been created within each of the newly formed Primary Care Networks which cover small groups of GP practices supporting 30,000 to 70,000 patients. This position will involve working across practices which have formed to create the Primary Care Networks. The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in General Practice.

The post holder will work as part of a multi-disciplinary team in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in GP practices.

The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within General Practice.

If necessary, the post holder will be supported to develop their role to become a non-medical prescriber.
