Job summary

GPA Federation is excited to be recruiting a Paramedic for Grand Union Primary Care Network. This is a great time to join Primary Care, providing a specialist Advanced Paramedic resource for health care professionals and patients, working in collaboration with other members of the growing multidisciplinary Primary Care teams.

You will be working both within PCN GP practices and the community, as an autonomous, accountable, Advanced Paramedic Practitioner, in the provision of a holistic approach for individuals.

You will be part of a growing team of Additional Roles staff, with dedicated leadership and support. We offer an NHS pension and generous annual leave allowance, and are a caring team to work for and with!

Main duties of the job

With a  minimum of 5 years post-registration experience and 2 years of consolidation as a band 5 NQP, you will assess, diagnose, treat, refer or signpost patients with undifferentiated or undiagnosed conditions relating to minor illness or minor injury. Using your advanced clinical skills, you will provide education to patients, promoting self-care and empower them to make informed choices about their treatment. You will be a qualified paramedic or specialist paramedic registered with the HCPC, with a minimum of 3 years paramedic work experience.

You will be motivated, reliable, committed to team working and self-development, with a positive can-do attitude and excellent interpersonal skills.

Job responsibilities

Primary Duties and Areas of Responsibility

1. Diagnosing and treating patients presenting with minor illness

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

Work from the PCN GP practices and within communities as an autonomous practitioner caring for patients and families.

To undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

Be able to:

o perform specialist health checks and reviews;

o perform and interpret ECGs;

o perform investigatory procedures as required, and

o undertake the collection of pathological specimens including intravenous blood samples, swabs etc.

Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing.

Provide an alternative model to urgent and same day GP home visit for the network and undertake clinical audits.

Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered.

Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Prescribe/ issue medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care.

2. Administration and Professional Responsibilities

Participate in the administrative and professional responsibilities of the PCN practices teams.

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.

Ensure the clinical computer system is kept up to date, with accurate details recorded and amended.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.

Attend and participate in PCN and Advanced Paramedic Practitioner meetings as required.
