Job summary
Main Responsibilities:Undertake minor ailment
clinics (primarily face to face appointments but including telephone triage if
necessary), with or without prior triage by themselves or another clinician. Perform chronic condition
reviews, inc QOF reviews.
Undertake home visits and
care home reviews.
Assess, diagnose, plan,
implement evaluate treatment/interventions and care for patients presenting
with an undifferentiated, undiagnosed problems.
Diagnose, manage both
acute and chronic conditions, with relevant clinical support.
Initiate and interpret
tests/investigations as appropriate
Work with patients to support compliance with and adherence to prescribed treatments
Prioritise health problems, intervene appropriately to assist patient in complex, urgent or
emergency situations, inc. initiation of effective emergency care
Support patients to adopt
health promotion strategies that promote healthy lifestyles, apply
principles of self-care
Record accurate data in
the patient records to an agreed standard.
Collaborate with other
members of the Primary Health Care Team including doctors, nurses and other
AHPs, accepting referrals and referring to them for specialist care.
Direct referral to and
liaison with consultants and other professional agencies.
Ensure a high standard of clinical practice is maintained by ensuring care is
based on current best practice, embarking upon further training if required and
continuing to update clinical knowledge.
Main duties of the job
The post holders
main responsibility will be to work within the clinical team to provide high
quality patient care. The PP
will provide clinical assessment and management for patients of all ages with
undifferentiated and undiagnosed problems seeking a primary health care service,
including minor illness face-to-face consultations, home visits, care home
visits and telephone triage.
The
PP will have ideally have experience of treating common minor illnesses in a
primary or urgent care setting. Training in this field is preferable but can be
provided after appointment.
The
PP will carry out home visits in the locality for patients who are housebound and
registered with the practice.
The
PP will have responsibility for an agreed number of care homes for which they
will be primarily responsible for clinical care, including weekly care home
rounds. The PP will work in partnership with the care home lead GP, clinical
team including practice pharmacists, community health and social care team
members and care home staff.

We have an friendly and dynamic team of 13 GPs, 4 pharmacists, 4 medical assistants, 2 paramedics, 1 physician associate,4 practice nurses. We are a training practice with 5 trainee GPs working within the team.
