Job summary
We are seeking an enthusiastic, motivated and experiencedPractice Nurse with a keen interest in diabetes to join our team. The ideal candidate will have experience managing long-term conditions and delivering high-quality, evidence-based care within General Practice. The role involves a mix of chronic disease management, preventative healthcare, and general treatment responsibilities.
Key duties include:
- Managing and delivering care for long-term conditions (e.g., asthma, COPD, diabetes, hypertension).
- Conducting cervical smears, contraceptive care, and baby immunisations.
- Providing routine treatment such as wound management, phlebotomy, and ECGs.
- Promoting health and self-care principles among patients.
- Delivering care to housebound patients as required.
- Providing vaccinations, including travel.

Main duties of the job
Deliver person-centred care based on evidence and in line with national and local guidelines.Deliver all immunisation programs and vaccinations the Practice participates inManage chronic diseases using care plans and recall systems.Participate in health promotion and screening initiatives.Communicate effectively with patients, carers, and colleagues, ensuring sensitive and clear information exchange.Maintain accurate patient records and comply with GDPR and confidentiality standards.Work within infection control and safeguarding guidelines.Actively participate in clinical governance, training, and quality improvement activities.Days are flexible, however a Friday is essential due to room space and PN cover.NO AGENCIES PLEASE - We do not use agencies to recruit

We are an established, forward-thinking General Practice based in Mickleover, Derbyshire, serving a patient population of approximately 13,000. Our clinical team includes 5 GP Partners, 3 salaried GPs, 1 ANP, 1 NP, 1 Practice Nurse, and 1 HCA and our adminstrative team are fantasic. We use the SystmOne clinical system and consistently achieve high QOF and other performance targets.

We require someone to hit the ground running and be part of a successful practice nursing team that fully integrates with the wider team.
The successful candidate will be self- motivated, autonmous within their role and truly care for our patients.
All of the team are looking forward to welcoming our new member of the team.
