Job Title: Trainee Care Coordinator / Care Coordinator
As a care coordinator you are the backbone that links our reception team with our clinical team, the care coordinator role is to carry out complex administrative duties and provide support to both our teams. It is a challenging and rewarding role which comes with responsibility and we are looking for applicants who are proactive and willing to take on a challenge.
The practice welcomes applicants looking for full time or part time work.
The Trainee Care Coordinator role is for applicants that have some or limited administrative experience that are eager to learn and build their skills.
The Care Coordinator role is for more experienced applicants that have experience in general practice/healthcare and/or in a administrative role.
The role the applicants are given will be based on experience and we will consider all applicants no matter their level of experience hence the two roles.
Main duties of the job
Patient support and advocacy
Serve as the point of contact,
advocate and informational resource for patients, care teams, family
/caregivers and community resources where indicated, responding with empathy
and respect.
Oversee
proactive routine management of patient care
Arrange appointments for patients
as directed by clinicians following identification of urgent and non-urgent
clinical need to assess diagnose and treat according to the patients health
needs.
Referral management
Manage all referrals on behalf of
the clinical team; ensure referrals are sent via the appropriate route with all
necessary supporting documentation, liaise with patients to book appointments
and manage rejected referrals.
Support multidisciplinary work
Coordinate and support delivery of
multidisciplinary team reviews (MDTs) within the practice and PCN, to include
management of the team diaries and arrangement/planning of team meetings,
completing actions and producing reports as requested.
Support medication optimisation
Work as an active member of the
Prescriptions Optimisation Team to ensure delivery of legal, safe, high quality
prescription management which reflects national and local policy and
priorities.
Collaborative working
relationships
Demonstrate use of appropriate
communication to co-operate with relevant stakeholders (including
patients, senior and peer colleagues, and other professionals).

between Camberwell and Brixton. The practice is based in the spacious and
purpose-built Akerman Health Centre. We are proud to support a diverse and

You will be joining our friendly and dedicated team of
Care Coordinators, who are supported by a team of Patient Services Officers, a Data and Operations
Lead, the Practice Operations Manager, and our co-Practice Business Managers.
The practice's clinical team is made up of 3 GP Partners, 7 salaried GP's, 1 Pharmacist Partner, 1 Practice Nurse, 2 Healthcare Assistants
and 3 Pharmacists, as well as Physiotherapists, Dietician, Health & Wellbeing Coaches, Mental Health
Practitioner, and Alcohol & Drugs Counsellor.
We are a teaching practice that trains clinicians at all stages
of training for the future NHS healthcare workforce. The practice is accredited
for teaching medical students by both Kings College London and St Georges
University of London. It is also accredited for training Foundation doctors and
General Practice trainees by National Health Service England London Deanery.

We are looking to appoint a care coordinator who will play a key role at the heart of our GP Practices, working alongside other PCN colleagues to deliver holistic coordinated care to our patient community.
- The post holder will be responsible for the co-ordination and delivery of specific projects to ensure the achievement of key health and welfare objectives.
- The post holder will work alongside other clinicians and as part of a multi-disciplinary team. The post holder will take responsibility for an area of work that will contribute to fulfilling the requirements of the Network Contract Directed Enhanced Service (PCN DES).
