Job summary

Would you like to work collaboratively within an established general practice team to meet the needs of patients presenting with a wide range of health conditions?

We are looking for an experienced clinician who will be the first point of contact for our patients with all types of health problems. You will be required to see patients face to face within the Practice and at times within the community, within care homes and via telephone triage. Meetings will form part of the role to include MDT meetings, significant event meetings and prescribing meetings.

If you are a qualified clinician, with experience of providing excellent patient care, plus experience of home visits, working within care homes and providing care to patients of all ages then we would love to hear from you.

You would be joining a welcoming and very busy single practice Primary Care Network (PCN) with over 30,500 patients.

Main duties of the job

The postholder will be the first point of contact for patients with all types of health problems. You will make decisions and apply appropriate clinical interventions to the presenting patient in the surgery, their own home, community or care homes. This will include initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. You will work autonomously but within the Practice's and NMC policies, protocols and guidelines, demonstrating safe clinical decision-making, high levels of critical thinking and holistic practice. You will generally be expected to act independently but will have access at all times to a GP for advice and support as necessary, for example in relation to prescribing decisions. You will participate and contribute in MDT meetings including significant event meetings and prescribing meetings etc. You must be able to apply appropriate assessment and management skills across the age range (children and adults)

We have an established team of 10 GP partners & a host of varied health professionals including 3.5 pharmacists assisting with medication reviews, experienced and well developed nurse and admin team, ANP's, 2 Paramedics undertaking home visits and care home reviews, Physiotherapist, Social prescribing link worker, care co-ordinators & excellent integration with community nurses and the voluntary sector.

You will form part of an existing team, working together to maintain a high quality service.
