We are looking for an experienced qualified nurse manager who is responsible for leading the delivery of general practice nursing to Newgate Medical Group practice population. You would be joining a welcoming and very busy single practice Primary Care Network (PCN) with over 30,500 patients.

You will lead and manage the nursing resource, working closely with support from the Practice Manager, Business Manager and GPs to deliver the Practice priorities.

Main duties of the job

As nurse manager you will be responsible for leading the delivery of general practice nursing to NMG practice population.

Your main duties will be split to include:

  • clinical practice: seeing patients presenting with a wide range of health conditions, working alongside community teams, visits if required, implementing & participating in vacc & imm programmes, identifying, diagnosing & managing treatment plans for patients, wound care, providing excellent patient care, to include presentation, communication & administration. Delivering a quality service, recognising & working within own competence & professional code of conduct as regulated by the NMC.
  • Managerial responsibilities: Managerial responsibility for organising & the delivery of the nursing service within NMG. Team leader & point of contact. Participating in Management meetings on behalf of your team. Conducting appraisals, organising team meetings, rotas, keeping records of training & encouraging teamwork within the Practice. Manage your teams budget, identify & manage risks, participate in audits, ensure all policies and procedures are up to date and followed.

We are a large Practice. We have an established team of 10 GP Partners & a host of varied health professionals including pharmacists, nurses, ANP's, PA, Paramedic, Physiotherapist, Social Prescribing link worker, care co-ordinators & a large admin team.

Job Summary

The post holder is responsible for leading the delivery of general practice nursing to Newgate Medical Group practice population. The focus of the role is a combination of patient facing and managerial responsibilities, focused on the organisational leadership and management of the delivery of evidence-based practice for patients seen by the nursing team. Within the Practice the post holder will act as an autonomous practitioner, responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. The post holder will work collaboratively within the general practice team to meet the needs of patients, manage, and oversee the development of relevant policies and procedures, and provide nurse leadership in appropriate areas within the Practice.

The patients are paramount in the leadership of the nursing team, any additional Practice and / or national campaigns will need the post holders full support as well as leading the team to work together successfully.

As team leader for the General Practice Nursing team, the post holder is accountable for the delivery of the nursing service within Newgate Medical Group. They will lead and manage the nursing resource, working closely with the Practice Manager, Business Manager and GPs to deliver the practice priorities. The post holder will also be expected to produce a holiday and relief rota working together with the nursing team to ensure service provision is maintained. The post holder will also have managerial responsibility for the efficient organisation of the treatment room and nursing support staff working for the Practice.

Key Responsibilities

Managerial

Organise the delivery of the nursing service with Newgate Medical Group.

Identify and prioritise key clinical activities across the team and manage performance of the team overall.

Act as team leader and point of contact for the nursing team, provide mentor leadership and support their personal development identifying and addressing any training needs.

Participate in Practice Team meetings, delivering the nursing agenda and organise direct team meetings.

Conduct regular appraisals with individuals of your nursing and support team.

Participate in clinical supervision for the nursing team, and act as nurse advisor to other members of the Primary Healthcare team.

Encourage and develop teamwork within the practice nursing team and between nursing team & other staff.

Participate in multi-disciplinary protocols and Patient Group Direction development.

Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff.

Help the practice operate in a cost-effective manner.

Identify and manage nursing care risks on a continuing basis.

Participate in practice meetings and practice management meetings, reporting progress as required.

Provide support and advice as required and monitor child protection training within the practice nursing team

Participate in audits and inspections as appropriate.

Work closely with the doctors and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets.

Develop nursing team specific policies and ensure that all Practice policies are fully implemented.

Maintain necessary occupational health records for clinical staff.

Create, manage, and apply main nursing and ANP rotas to System 1.

Oversee Infection Control Lead, for Care Quality Commission compliance. To be

responsible for ensuring the practice Infection Control policies are adhered to including the use of audits, action planning and risk assessments when needed and to have overall responsibility for staff education in this area.

Teaching and Mentoring

Promote a learning environment for patients, nurses, and other health professionals, providing teaching sessions as required.

Contribute to the planning and implementation of the teaching for existing staff, such as Medical students, Nursing students, GP registrars and Health Care Assistants within the Practice.

Clinical Practice

Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being.

Work alongside Community Teams to assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition.

Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition as appropriate.

Work with other healthcare professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan in line with national and local policies and practice needs.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.

Work with patients to support adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote patients to live healthily and apply principles of self-care.

Support and manage health needs of women presenting for family planning and cervical cytology consultations.

Recognise, assess, and refer patients presenting with mental health needs in accordance with NICE and other authoritative guidance for Mental Health.

Implement and participate in vaccination and immunisation programmes for both adults and children.

Advise, support, and administer vaccinations where appropriate for patients travelling abroad.
