This post is part time 20-25 hours
The Advanced Clinical Practitioner will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, acting within their professional boundaries.
The role includes:
- providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
- demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the practice
- demonstrate critical thinking in the clinical decision making process and work autonomously within their scope of experience and qualifications but will have the support of the practice
Main duties of the job
The post holder should be able to demonstrate that they are working within HEE Multi-Professional Framework for Advanced Clinical Practice demonstrating core capabilities within the four pillars of the framework and effectively role model the values of being an ACP.
