Job summary
The Clinical
Coder is primarily responsible for supporting the Data Quality Team to ensure that all
external and internal patient correspondence is acted upon at the appropriate
level and accurately coded in the patients medical record. A sound
professional working relationship will be required with the GPs and staff
working within the Practice.
The role includes:
Familiarity with
Docman and EMIS Web clinical software systems
Ensuring that all
incoming patient correspondence is scanned, acted upon and correctly coded to ensure the
integrity of patient records
Accurately
summarise patient records
Main duties of the job
The post
requires high levels of organisation and attention to detail as well as the
ability to exercise tact, discretion and initiative. The ability to work under pressure and as
part of a team are equally important.
The Practice
will support continuing education and professional development in the form of
attendance at study days, courses and with in-house training, as deemed
appropriate for the benefit of both the individual and the Practice.

- Identifying relevant clinical information
documented in patient correspondence, code appropriately and complete
any action required
- To scan all patient related post onto the system using Docman software and to file this information accurately to the patient's medical record using pre-set options
- To correct errors in filing and ensure corresponding records are amended accurately
- Assess correspondence and deal with as
appropriate and/or pass to appropriate colleagues via workflow process,
ensuring coding and organising of all relevant information
- Follow practice protocols to ensure
consistent and high quality care is provided to our patients including
data accuracy
- Code with additional free text, if
appropriate, the clinical information on to the patient record using
practice templates and EMIS Web software.
- Ensure Problems are recorded appropriately
and rationalised when required
- Summarising of patient healthcare records,
within specified timescale
- Deal with queries relating to coding
- Assist with
auditing process with the Workflow GP Lead
To attend
internal and external meetings as appropriate
To keep up to
date with mandatory and role specific training as well as attend training
courses and other events relating to the role
Helping to
solve day-to-day issues with IT, systems, workloads, etc as they arise
To keep
up-to-date with the Policies and Procedures of the organisation
Undertake any
other duties appropriate to the role
During absence
of colleagues provide essential cover and carry out any other appropriate
