Job summary

Health Connectors support people to take proactive steps to improve the way they manage their physical health and mental wellbeing, based on what matters to them. They use a range of health coaching techniques to help people to develop their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life.

Health Connectors are experts at working with individuals to help them to tap into and activate support networks in their communities and linking them to practical assistance and support.

Main duties of the job

The postholder will work in the West Mendip PCN area delivering person-centred care and support, as part of the Health Connections team. Health Connectors support people to better manage their health, wellbeing and social welfare through a combination of one-to-one support, groups and community projects.

They are a key part of the PCN multidisciplinary team; this involves working closely with health, social care and community professionals to provide personalised care to patients and their carers, and to promote this approach within the PCN.

About us

The successful candidate will be based in a local cluster of General Practices as part of West Mendip Primary Care Network (WMPCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

West Mendip PCN is an NHS collaboration between the 5 practices of Street, Wells and Glastonbury. We have a population of 54,000 patients and are working together to provide additional and extended services to our patients.

Primary Duties & Areas of Responsibility

Social prescribing: helping people to improve their health, wellbeing and social welfare by connecting them to community services and projects. By increasing peoples active involvement with their local communities, personal and community resilience is strengthened, and health and wellbeing inequalities can be reduced

Deliver projects within the community, including Talking Cafés, Talking Benches and the Healthy Lifestyles programme, face-to-face or online as appropriate

Provide one-to-one and group health coaching support for people with one or more long-term health conditions, based on what matters to them, with the aim of empowering people to manage their own health, and improve their health outcomes

Manage and prioritise a caseload of patients. Understand when it is appropriate or necessary to refer people back to, or on to, other health professionals/agencies

Support the promotion of the Health Connections directory/website, resources, projects and one-to-one support across the locality

Identify gaps in local service provision through speaking to patients/communities and feed back to line manager

Attend and contribute to team, GP practice and PCN meetings and events as required by the service

Participate in regular supervision, reflective practice and continual learning

Experience

Essential

  • Experience of supporting people, their families and carers in a related role (including unpaid work)
  • Experience of working with people one-to-one using behaviour change, motivational interviewing and goal setting tools
  • Experience of working with the VCSE sector (in a paid or unpaid capacity)
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)
  • Experience of supporting people with their mental wellbeing, either in a paid, unpaid or informal capacity
  • Experience of data collection and using tools to measure the impact of services

Desirable

  • Experience running and facilitating groups
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
