The aim of the Health and Well-being
Coach will be to develop collaborative relationships and to empower people to
take control of their own health and wellbeing.
The post holder will be expected to play
a dynamic role in supporting patients to adopt healthier lifestyles and to
coach and mentor individuals to achieve personalised health care
objectives. In undertaking this role the
successful candidate will be required to demonstrate effect team working and a
strong commitment to delivering the Abbey Health PCN agenda to deliver
excellent care for all patients within the PCN.
The successful candidate will be empathetic,
reflective, self-aware, and enjoy working with a wide range of people. They
will have good communication and negotiation skills and able to provide a high
quality, non-judgemental service. They will be proactive with a flexible
attitude, a good team player and committed to developing themselves in this
skilled role. Health and Wellbeing Coaches will have access to ongoing
supervision, skills development, and support so they are able to continue to
develop and gain experience.
The successful candidate will work under the supervision of an
AHP Clinical lead, providing patient care at our PCN sites.
Primary Duties and Areas of Responsibility
The main duties
and responsibilities will be to:
Provide one-to-one and group health coaching support for people
with one or more long-term conditions, based on what is important to them, with
the aim of: improving peoples levels of activation, empowering people to
manage their own health and improve their health outcomes.
Develop collaborative relationships and work in partnership with
health, social care, and community and voluntary sector providers and
multi-disciplinary teams to holistically support patients wider health and
well-being, thereby contributing to the reduction of health inequalities.
Provide education and specialist expertise to PCN staff,
supporting them to improve their skills and understanding of personalised care,
behavioural approaches and ensuring consistency in the follow up of peoples
goals with MDT input.
Work with the GPs and other primary care professionals within the
PCN to identify and manage a caseload of patients.
Be the first point of contact at GP practices where coaching
services will be delivered, and to receive and manage referrals.
Where required and as appropriate, refer people back to other
health professionals within the PCN, in accordance with the needs, priorities
and any urgent support required by individuals.
Using existing IT and MDT channels to screen patients, with an aim
to identify those that would benefit most from health coaching
Provide personalised support to individuals, their families, and
carers to support them to be active participants in their own healthcare.
Facilitate and coach patients, both individually and in groups, to
manage their own health and wellbeing and live independently through:
coaching and motivating patients through multiple sessions to
identify their needs, setting goals, and supporting patients to achieve their
personalised health and care plan objectives;
providing interventions such as self-management education and peer
support;
supporting patients to establish and attain goals that are
important to the patient;
supporting personal choice and positive risk taking while ensuring
that patients understand the accountability of their own actions and decisions,
thus encouraging the proactive prevention of further illnesses;
working in partnership with the social prescribing service to
connect patients to community-based activities which support them to take
increased control of their health and wellbeing; and
increasing patient motivation to self-manage and adopt healthy
behaviours
Work with patients, both individually and in groups, to help them
understand their level of knowledge, skills and confidence when engaging with
their health and well-being and subsequently supporting them in shared
decision-making conversations.
Utilise health coaching skills to support people with lower levels
of activation to develop the knowledge, skills, and confidence to manage their
health and wellbeing, whilst increasing their ability to access and utilise
community support offers.
Work as part of a Multi-Disciplinary Team, participating in
progress meetings, ensuring that the MDT is kept fully up to date with progress
reports.
Actively engage and support the design and promotion of health
promotion campaigns.

Underpinning expectations:
The post holder is accountable to the PCN Clinical Director. They
are professionally responsible for their own actions and for the actions of
others under their supervision.
The Post Holder
is expected to:
Adhere to PCN policies
and procedures and those of the constituent practices where relevant

protection regulations, data sharing and respecting confidentiality of
patients, colleagues and the Primary Care Network
Undertake a
Disclosures and Barring Service (DBS) application
Be committed to

and professional development in order to maintain their skills

and diversity in a non-discriminatory way
To travel
independently between work sites and to attend meetings hosted by other
agencies.
Confidentiality:
The post holder must maintain the
confidentiality and security of information about patients, staff and the business
of the PCN and both partnerships in accordance with the Data Protection Act
2018 and Caldicott principles.
Health & safety:
The post-holder will support and
implement on the full range of promotion and management of their own and
others health, safety and security as defined in the practice Health &
Safety policy, the practice Health & Safety manual, and the practice
Infection Control policy and published procedures.
This will include (but will not be
limited to):
Using personal
security systems within the workplace according to Practice / PCN guidelines.
Identifying the
risks involved in work activities and undertaking such activities in a way that
manages those risks across the business.
Making effective
use of training to update knowledge and skills and initiate and manage the
training of others.
Using appropriate
infection control procedures, maintaining work areas in a tidy and safe way and
free from hazards, and initiation of remedial / corrective action where needed.
Keeping own work
areas generally clean, identifying issues and hazards / risks in relation to
other work areas within the business, and assuming responsibility in the
maintenance of general standards of cleanliness across the business in
consultation (where appropriate) with other site facility managers
Undertaking
periodic infection control training (minimum annually).
Demonstrate due
regard for safeguarding and promoting the welfare of children.

maintaining a record of own personal and/or professional development

skill and activities to others who are undertaking similar work
Training may need
to be undertaken outside of normal practice hours, and off site.
